Cable TV is a term that has been around for many years. In the US, cable TV has become the generic term for pay TV of any type.
In the UK cable TV refers to the cable TV operators that emerged in the 1980s and 1990s. These companies started digging up the roads and laying fibre-optic cable to people's homes in order to be able to provide home phone services, digital TV and more recently broadband services to consumers. So unlike Sky, which delivers it's TV service via satellite, cable TV is delivered via fibre optic cable.
